A painter who fell from a ladder at the workplace last week died in the hospital.
The man in his 60s was critical injury while using an A-Frame ladder on a site in Mandurah south of Perth last Thursday.
He was rushed to the hospital, where he lost his fight for life on Monday.
A Worksafe research into the tragedy is now underway.
Commissioner Sally North has shown her participation to the family and friends of the man.
It was not the only workplace-related autumn last Thursday.
Worksafe started a separate investigation after a young tradia fell nine meters from a community center in the suburb of Perth of Brentwood last Thursday.
The man, 18, did the roofing in the center before he fell through the roof and on the sports bench below.

The construction, forestry and maritime employee union has renewed the calls for the state government and the building authorities to improve the safety guidelines after the latest incidents.
Three fatal incidents with regard to employees who fell through fragile roof materials or roof openings were registered in WA last year.
