An 'fit and healthy' mother who died alongside her premature daughter was killed by a blood clot in her lungs, an investigation has been told.
Mellodie-Ocean Jarman, 19, was 33 weeks pregnant when she died in the Royal Bolton Hospital in Greater Manchester on 2 February, little more than one day to complain about feeling faint and dizzy during a routine pregnancy scan.
Doctors gave her baby daughter through emergency section and fought to save her premature little girl, but she also died after getting her last breathing agencies in her mother's arms.
Their deeply sad family thanked medical staff in the Royal Bolton Hospital for their efforts to try to save both the lives of Mellodie and Baby Athenas.
'We would like to express our gratitude to the Bolton team of the Emergency Department for Working so hard to save Mellodie, and to the neonatal unity and all the team on team service that tried to save from Athena on Saturday evening and Sunday for trying to save Athena, “said in a statement.
'Your love, caring and friendly support was very necessary, and you did everything to make us feel safe.
'The team of the Mortuary and our funeral workers are so understanding and takes care of the whole family.
'It was of the utmost importance to see that this event touched everyone involved, and as much as we are struggling to raise this whole situation, we can see that all involved are greatly affected.
“So, out of the ground of my heart, we would like to thank you that you are friendly and are caring at a time in life that is really needed.”

Her sad partner Daniel Darbyshire, 18, said that he was “lost and deeply sad” by the death of his partner and daughter.
He said: 'We were both excited to become partners and had our entire life to look forward to.
“I didn't only lose my partner and daughter. I lost my world, my dreams and my safety blanket. '
Mellodie's mother, Justine Ryan, 52, said: “She was my world and blessed me with my first granddaughter.
“She was funny and beautiful and would absolutely speak her thoughts.”

The young mother, from Atherton in Greater Manchester, complained about feeling faint and dizzy on the way to a routine pregnancy scan on Friday 31 January

The tragedy came little more than a day after Mellodie complained that he felt weak and dizzy during a routine pregnancy scan on January 31 and was sent home with medication and antibiotics.
A doctor said she had a low iron level and prescribed her liquid iron and antibiotics before the couple, who had been together for more than two and a half years, came home.
Daniel added: 'The day after she was good all day, we watched television and took pictures, just messing around. She seemed fine.
“Then, on Saturday evening she had the same weather and said she couldn't hear it and that she could see white, as she did on Friday, and that she had trouble breathing.”
An ambulance was called, but the teenager fell unconscious and stopped breathing before he collapsed in her mother's arms.
Paramedics desperately tried to breathe new life into her resuscitation and she was rushed to the Royal Bolton Hospital.
Doctors carried out an emergency C section and Mellodie is birth to Baby Athena-Pearl seven weeks early, at 12.32 pm on Sunday.
Tragically enough, Mellodie died about 80 minutes after the operation, while the small Athena-Pearl died hours later.

But on January 31, Mellodie complained about feeling weak and dizzy. She died days later, on 2 February. Her little girl, born early for seven weeks by emergency section, died hours later

Chef Daniel, from Leigh in Greater Manchester, told how he had placed his little baby girl in the arms of her deceased mother so that she could take her last breath with her mother.
He then pushed his daughter back from the mortuary in a pram and the family was able to bathe and baptize her.
After the double tragedy he said: 'I feel lost, I am not sure what to do.
'Mellodie would normally give me insight into what I have to do and keep life as a structure, now it's gone.
“At the moment I just get lost. The big word is actually lost and broken, really. '
Health bosses who supervise the Royal Bolton Hospital said that an investigation into the double death tragedy has started.
Dr. Francis Andrews, medical director at Bolton NHS Foundation Trust, said: 'Our sincere condolences are with the family and friends of Mellodie-Ocean and Athena-Pearl on this incredibly difficult time, and we do everything to support them.
“As with any unexpected death, we will conduct a complete and thorough investigation and will keep the family informed as it progresses.”
