Most prescriptions in the United States are handled by one of three companies: CVS Caremark, Express Scripts or Optum Rx. These pharmacy benefit managers act as a middleman between the pharmaceutical companies that make the medications and the insurance plans that pay for your prescriptions.

These relationships have been in the news because of the high cost of prescription drugs. We would like to know what your experiences are with these companies, for example whether your medicines were reimbursed and how much you paid for them.

We are also interested in the experiences of pharmacists and doctors about their experiences and those of their patients.

We read each answer to the questionnaire and then reach some of the respondents we want to know more about. We will not publish any part of your response without further communication with you. We do not share your contact information outside the Times newsroom and will only use it to contact you.
