A TRAGIC new detail has emerged after the deaths of a father, his two daughters and sister-in-law in a suspected murder-suicide.

Jasmin Kuczynska, 12, and eight-year-old Natasha Kuczynska were discovered at home at Costessey, Norwich, on January 19th.

Father Bartlomiej Kuczynski, 45, and the children's aunt Kanticha Sukpengpanao, 36, were also found following a 999 call.

An investigation into the fourfold tragedy heard today how Natasha and Jasmin's bodies were identified by a neighbor at the hospital.

Norfolk coroner Samantha Goward appeared to cry as the heartbreaking details were revealed.

The hearing was told that Natasha's preliminary medical cause of death was recorded as “sharp force injuries to the neck”.

Her older sister died of an “incised neck wound”, while Bartlomiej's cause of death was given as a “stab wound to the neck”.

Kanticha, who was visiting from Thailand, died of “stab wounds to the neck”. court was told.

The coroner adjourned the hearing for a preliminary investigation judgement on April 29.

She also ordered a mental health report health confidence after Bartlomiej came forward undergo an assessment before the tragedy.

Norfolk Police confirmed that the deaths of Jasmin, Natasha and their maternal aunt are being treated as homicides.

The father's death is not considered suspicious and officers are not looking for anyone else in connection with the killings.

The Independent Office for Police Conduct (IOPC) has launched an investigation into the quadruple tragedy previous contact with Bartlomiej.

He was later discovered close to his home and taken to Norfolk and Norwich University Hospital to undergo a mental health assessment.

“He was advised to seek medical advice but police were not present.

“Around 7am, police received a call from a dog walker who was concerned about the welfare of people in the house.

“Officers attended shortly afterwards and tragically found the man, a woman and two children dead.

“Following a mandatory referral from police, we have decided to investigate and will investigate whether the manner in which police handled the contact they had with the man was appropriate and in accordance with policy, training and procedures of the police.”

How mystery still exists about quadruple tragedy

More and more questions are being raised about how the four deaths occurred after it emerged that father Bartlomiej had called 999.

Despite warnings from officers about his mental state, he was told to seek medical advice.

Why didn't the police respond to the call?

Norfolk police have still not confirmed why they did not send officers to the house when Bartlomiej raised his concerns.

He told 999 he was “concerned” about his mental state and was “confused”.

The IOPC is now investigating why that call was ignored.

What is the 'missing person' investigation?

In a second IOPC referral, it was revealed that police had previously been in contact with the family before the tragedy.

Officers confirmed this was related to the December 14 missing person investigation.

Bartlomiej was reported missing but later found wandering in the woods.

Why did the police only respond to the second call?

An hour after the initial 999 call, the alarm was raised when a dog walker raised concerns for the welfare of those inside.

It is not yet clear why that call was responded to, but the father's was ignored.
